Countryside Public Health is a five county Community Health service agency serving the counties of Big Stone, Chippewa, Lac qui Parle, Swift and Yellow Medicine in western Minnesota. There are offices located in Ortonville, Montevideo, Madison, Benson and Granite Falls.
It is the Philosophy of Countryside Public Health to support all individuals to reach and/or to maintain an optimal level of well being in their place in society. Such support shall be available to all members of the area. The service will strive to promote, plan, cooperate, and coordinate with other agencies and institutions to develop that optimal level, and to become a leader in the development of a comprehensive community health program
The Mission of Community Health Services is to bring people together to create a healthy future for all Minnesotans. The Community Health Board, governed by a joint powers agreement is composed of a county board member from each of the counties, (Chippewa has 2 board members), and a lay representative from each county.
Countryside Pubic Health seeks to promote good health, to improve conditions in the social and physical environment that hinder optimal health, and to work toward prevention of illness and injury. For the most part we work with families and those non-hospitalized individuals that have heath problems that affect the community as a whole.

Countryside Public Health Community Health Board regularly schedules meetings the 2nd Wednesday of each month, at 2:30 pm at the Chippewa County Family Services Board Room, Montevideo MN. If you are interested in attending, please call 320-843-4546 as cancellations are possible.
