Countryside Public Health is a five county Community Health Service Agency serving the counties of Big Stone, Chippewa, Lac qui Parle, Swift and Yellow Medicine in western Minnesota. Our offices are located in Ortonville, Montevideo, Madison, Benson, and Granite Falls.
