Services in Your Home or ApartmentServices in your home or ApartmentHome care providers may provide services in your home or apartment. These services are designed to help keep you safe and healthy and in your own home. Services could include nurse visits, home health aide, Meals on Wheels, blood draws, homemaker, Personal Care Attendant, medication monitoring or Medical Alert. Long Term Care Consultation Getting services sooner rather than later may help the elderly remain independent and in their homes longer. Each Minnesota county provides a free Long Term Care Consultation in the home to help assess an elderly person’s situation and suggests services to assist them. It may be setting up rides to church or to the doctor, ordering home delivered meals, light housekeeping, home health aid assistance for personal cares or skilled nursing visit. Call your County Human Services to schedule this consultation service. Family Services Other Services Other services to provide safely in the home include medical supplies like raised toilets and grab bars. Prairie Five offers assistance in some home modifications for seniors. Emergency alarms or devices can be worn by an individual and set off in case of a fall, or emergency are also available. The Granite Falls Living at Home/Block Nurse program The living at Home Block/Nurse Program is a community program that draws upon the professional and volunteer services of local residents to provide information, social and support services, nursing and other services for their elderly neighbors to help them to remain independent in their homes. It is available to any resident of Granite Falls who is 65 years of age. Please contact the Living at Home/Block Nurse program if you would like to be a volunteer.
