Child & Teen Checkups What are Child & Teen Checkups?

Child & Teen Checkups (CTC) are checkups for children and teens, birth to 21 years of age, who are enrolled in Medical Assistance or MinnesotaCare

Where do I get Child & Teen Checkups?
  • Your doctor or clinic
  • Call Countryside Public health for information on CTC check ups.


Child & Teen Checkups include...
  • Information about good physical health
  • Information about thoughts, feelings and relationships with others
  • Time to ask questions and get answers about your child's health and development
  • Complete physical exam
  • Shots
  • Hearing check
  • Vision check
  • Lab tests
  • Checks on growth and development
  • Referral to the dentist
Use this health checkup for Head Start, WIC, school or daycare or athletic physicals. Be sure to take any forms you need with you.



When should my child get Child & Teen Checkups?
  • Between birth and 1 month
  • At 2, 4, 6, 9, 12,15 and 18 months
  • At 2, 3, 4, 5 and 6 years
  • Every two years after age 6
